I provide ongoing educational support throughout the life of each case, from initial assessment and planning through to monitoring, review, and long-term outcomes. Depending on complexity, my involvement may continue over extended periods.
I participate in multidisciplinary team (MDT) meetings and maintain ongoing liaison with case managers, therapists, and other professionals. Within MDT settings, I contribute educational analysis and recommendations to ensure that educational planning is fully aligned with rehabilitation goals, care planning, and wider case objectives.
My assessments and reports are designed to be practical, actionable, and directly usable within case management and legal frameworks. I focus on ensuring that educational recommendations are clearly linked to functional outcomes, progression, and placement decisions, supporting integrated decision-making across the MDT.
